Health plans and Federal and State Government agencies are responsible for reporting to the HIPDB certain final adverse actions taken against health care practitioners, suppliers and providers.

There are three ways of submitting reports to the HIPDB:

• Interactively via the Internet using the Integrated Querying and Reporting Service (IQRS).
• Using an XML transaction file submission program, the Querying and Reporting XML Service (QRXS).
• Using a proprietary transaction file submission program, the ICD Transfer Program (ITP).
